Reviews pharmacist and pharmacy responsibilities for medication error prevention. Describes techniques for the promotion of patient safety in pharmacy. Discusses continuous quality improvement as an application of root cause analysis and failure mode and effects analysis.

Purpose

To meet the continuing education requirement of two hours in medication error prevention every two years.
