Course Description
Reviews pharmacist and pharmacy responsibilities for medication error prevention. Describes techniques for the promotion of patient safety in pharmacy. Discusses continuous quality improvement as an application of root cause analysis and failure mode and effects analysis. Describes the aviation model of error prevention and applies that model to health care. Case studies of pharmacy error are included.
Purpose
Regulatory compliance with state board of pharmacy disciplinary requirement, or in anticipation of disciplinary action.
