Description
This program is designed to educate readers on the basic elements of human error and how they apply to the practice of pharmacy. Theories of human error are discussed, followed by a discussion of factors that can influence error capture, production and prevention and error analysis techniques like Root Cause Analysis (RCA). Finally, the course concludes with a summary of main ideas in order to become more “error-aware.”

Kraig L. Schell is Associate Professor of Psychology and the director of the Masters program in Industrial-Organizational Psychology at Angelo State University in San Angelo, TX. He has a Ph.D. in psychology from the University of Cincinnati, with emphases in applied and organizational psychology. He specializes in research and consultation around issues of work and consumer safety, human error, motivation and attitudes. He is the editor of a continuing education series on psychology in pharmacy practice, has served as an expert witness in cases involving medication errors, and is the author of over 60 professional publications and presentations.

David Brushwood is Professor of Pharmaceutical Outcomes and Policy at the University of Florida College of Pharmacy. A graduate of the schools of pharmacy and law at the University of Kansas, Professor Brushwood practiced both professions prior to entering academia. Professor Brushwood has developed error prevention programs for several pharmacy chains. His research interests are in the areas of regulating for outcomes, medication error prevention, and pain management policy. He has received grant funding from numerous agencies including the Robert Wood Johnson Foundation, the National Institutes of Health, and the National Community Pharmacy Foundation. He was twice selected as a Mayday Scholar in Pain Policy by the American Society of Law, Medicine & Ethics. Professor Brushwood is a frequent contributor to pharmacy journals and to law journals. He has developed, and he currently coordinates, online part-time graduate programs leading to the Master of Science in Pharmacy degree within seven separate areas of concentration.

CE Accreditation
The University of Florida College of Pharmacy is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. All five modules of this course must be completed to fulfill the requirements of the Florida Board of Pharmacy. The UAN for this course is 0012-9999-12-111-H05-P. The course is a Knowledge-Based activity and is accredited for 8 hours or 0.8 CEU of continuing pharmacy education credit. The release date for the course is 02/02/2012 and the expiration date is 02/02/2015.
Pharmacist Evaluation, Post-Test, and Statement of CPE Credit
The purchase price of this program via the RxLaw website includes the cost of accreditation. This course is approved by the Florida Board of Pharmacy for Medication Error (Misfills) and for General Pharmacy Credit. Statements of CPE Credit will be issued by University of Florida College of Pharmacy via email upon completion of the program evaluation and the post-tests with a score of 70% or higher. CPE credit will be reported to CE Broker. No partial credit will be given and you must complete all 5 modules to earn Florida Board of Pharmacy Misfills credit. Beginning in the first or second quarter of 2012 CPE credits will be reported directly to CPE Monitor and CE Broker. If you require a Statement of CPE Credit to comply with Board of Pharmacy requirements include that information on your answer sheet. |
